NOMINATIONS FOR THE 2025-2026 AWARD OF EXCELLENCE

Do you know someone who has made a difference in WISCPHR? Someone who has advanced the field of cardiac and/or pulmonary health and rehabilitation in Wisconsin?

The Award of Excellence is given in recognition of outstanding contribution by an individual in the field of cardiac and/or pulmonary health and rehabilitation. This award is intended to identify an individual who is motivated to improve the lives of patients and their families as well as the practice of cardiac and/or pulmonary rehabilitation. This is an individual who leads by example and who is a role model for professionals as well as patients.

Award Criteria:
Required:

  1. Actively working in the field of cardiac and/or pulmonary rehabilitation
  2. Involved at the regional and state level
  3. Is currently or has been a member of the WISCPHR Board of Directors (elected officer or Regional Rep)
  4. Is currently or has served on or chaired at least one WISCPHR committee
  5. Is currently or has served/lead various WISCPHR projects such as Day on the Hill, legislative/ reimbursement projects, etc.
  6. Current WISCPHR/AACVPR Member

Preferred:

  • Currently or was recently involved in AACVPR (committee, task force or other project) or other professional
    organizations that have a similar mission as WISCPHR (ACSM, PCNA, WI Cardiovascular Health Alliance,
    ALA, AHA, AARC)
  • An example of professional leadership through involvement in administrative, political or community pursuits
    that further the mission and goals of WISCPHR and cardiac or pulmonary rehabilitation

Past award winners (2024 to 1996) include Abigail Ahlskog, Samantha Rank, Kim Radtke, Colleen Voll, Heidi Grafft, Shana Steele, Lance Maerz, Tammy Riebe, Thorne Wittstruck, Sandy Zemke, Heather Grant, Carol Boe, Sue Kindschi, Cindy Ostrem, Kim Beyer, Tammi Smith, Carolyn Verhage, Barb Scotty, Bonnie Anderson, John Porcari, Roseanne Brandau, Barb Fagan, Mark Vitcenda, Sue Fuhrman, Kathy Menard-Rothe, Tracy Herrewig, Jean Einerson and Carol Winkler.

A committee of past award winners will review all nominations to select this year’s Award of Excellence winner.

The award is presented each year at the WISCPHR annual meeting. This year the award will be presented on April 17th-18th in Oshkosh, WI.

NOMINATIONS FOR THE 2025-2026 HONOR OF CLINICAL DISTINCTION:

Click here to nominate an individual for the Award of Honor of Clinical Distinction

The Honor of Clinical Distinction is given in recognition of an outstanding clinician in the field of cardiac and/or pulmonary rehabilitation. This award is intended to identify an individual who is highly effective in improving the outcomes of their patients by applying superlative clinical vigilance and motivational techniques. The candidate should also be considered a resource for program guideline clarification and other regulatory bodies.

Criteria
Required:

  1. Working in the field of cardiac and/or pulmonary rehabilitation for a minimum of 3 years 
  2. AACVPR/WISCPHR joint affiliate member
  3. Works within an AACVPR certified program
  4. Holds professional certification
  5. Active participant in AACVPR program certification process

*Nominator required to provide a paragraph on why the individual deserves this recognition and should provide the above information. Nominator should also have direct work experience with the nominee.

The number of honorees is unlimited.

The above will be read during the lunch/business meeting at the annual conference, with the honorees announced individually.

Recipient will be presented with a certificate.

Letters of recognition will be sent to work managers/administrators/program medical directors.

Each year at the Annual Conference, WISCPHR awards the "Award of Excellence" to members who are outstanding professionals and who have served the Society as mentors and leaders.

Presidential Citation & Recognition Award

Each year at the Annual Conference, the outgoing WISCPHR President has the opportunity to formally recognize an individual the "Presidential Citation & Recognition Award" who has made significant contributions to the president's term and the association as a whole.

Distinguished Service Award

Each year at the Annual Conference, the WISCPHR Executive Committee has the opportunity on any given year to award the "Distinguished Service Award" to an individual whose leadership, ideas, and committee work have significantly benefited WISCPHR as an organization.
